That is not carelessness.

Traditional cyber cover is built around a long proposal form. It asks clients to describe a risk they often cannot see clearly: how long they could continue trading, what an attacker could reach, and whether every account is properly secured. The person completing it is usually wearing several hats and is not an IT specialist. As a result, the form is often completed quickly and gaps are filled with assumptions. 

Then those answers are relied on at claim time.
